  • It is required for you to read through this information prior to submitting this application.
  • After you have submitted this application, you will need to meet with a faculty advisor first before taking this form to the advisement center to declare the minor. You will meet with an advisor based on last name. Check the appropriate professor, then after submitting your application make an appointment to meet with them. The professor will also be sent an email notifying them of your application. *Matt Baker is not available during the summer, please see another one of our faculty advisors.
  • Print this form from the confirmation email you will receive. Your faculty advisor will sign before you take this document to the advisement center.
