Internship Master Agreement. Any off-campus internship provider must sign this agreement before a student begin an internship. Most of our providers already have a signed agreement on file with the Internship Office. To see if a provider has signed the Internship Master Agreement, check the “Current Experience Provider Database” on the Internship Office Web site.
Student Agreement. Fill out this form and have it signed by your supervisor. Return it to the editing internship coordinator. You must turn in the form before you begin an internship.
